Respiratory Tract Infections (RTIs)
Overview
Respiratory tract infections (RTIs) are illnesses affecting the nose, sinuses, throat, airways, or lungs. They include common illnesses such as colds, influenza, COVID-19, RSV infection, acute bronchitis, and pneumonia. Many are caused by viruses and improve with time and supportive care, although some infections can become more serious or require specific medical treatment.¹˒²
At Tangible Health, we take an individualized approach by looking at your symptoms, duration of illness, breathing, fever, medical history, medications, nutritional needs, sleep, and risk factors for complications. For mild and uncomplicated infections, care can focus on comfort and recovery. When symptoms suggest pneumonia, significant breathing difficulty, or another potentially serious infection, appropriate medical assessment takes priority.
What Are Respiratory Tract Infections?
Respiratory tract infections are commonly grouped according to the part of the respiratory system affected.
Upper respiratory tract infections involve the nose, sinuses, and throat. Examples include:
The common cold
Viral sore throat
Some sinus infections
Influenza and COVID-19 when symptoms are primarily upper respiratory¹˒³
Lower respiratory tract infections involve the larger airways or lungs. Examples include:
Acute bronchitis
Bronchiolitis
Pneumonia¹˒⁴
Viruses cause many respiratory infections, including influenza, COVID-19, RSV, rhinoviruses, and other common respiratory viruses. Bacteria can also cause respiratory infections, including some cases of pneumonia, sinusitis, and strep throat.¹˒⁴ Different infections can cause very similar symptoms, so a cough, fever, sore throat, or congestion alone does not always reveal which virus or bacterium is responsible.
Common Signs & Symptoms
Symptoms depend on the infection and which part of the respiratory tract is affected.
Upper respiratory symptoms may include:
Runny or stuffy nose
Sneezing
Sore or irritated throat
Cough
Sinus pressure or congestion
Headache
Lower respiratory symptoms may include:
Persistent or worsening cough
Cough with mucus
Wheezing
Chest discomfort
Shortness of breath
Faster or more difficult breathing⁴˒⁵
General symptoms may include:
Fever or chills
Fatigue or weakness
Muscle or body aches
Headache
Reduced appetite
Feeling generally unwell¹˒³
The duration also varies. A common cold often improves within one to two weeks, while the cough from acute bronchitis can persist for several weeks even after the infection itself is improving.⁴˒⁶ Symptoms such as increasing shortness of breath, chest pain, persistent high fever, or significant weakness can suggest a more serious lower respiratory infection and warrant medical assessment.⁵
Causes & Contributing Factors
Viral Respiratory Infections
Viruses cause most uncomplicated respiratory tract infections. Common examples include influenza, COVID-19, RSV, and the many viruses responsible for the common cold.¹˒² Because antibiotics act against bacteria rather than viruses, they do not treat colds or influenza and are usually not helpful for uncomplicated acute bronchitis.⁶˒⁷
Bacterial Infections
Bacteria are responsible for some respiratory infections, including certain pneumonias, strep throat, and selected cases of sinusitis. Antibiotics may be important when a bacterial infection is diagnosed or strongly suspected.⁵˒⁷ The presence of coloured mucus alone does not necessarily mean an infection is bacterial or that antibiotics are required.
Exposure & Transmission
Respiratory viruses can spread through respiratory particles released when people breathe, speak, cough, or sneeze, as well as through close contact and contaminated hands or surfaces in some circumstances. Staying home when sick, improving indoor ventilation, hand hygiene, and using appropriate masking in higher-risk settings can help reduce transmission.²˒⁸
Risk of More Severe Illness
The risk of complications varies according to the infection and the individual. Older adults, infants and young children, pregnant people, people with weakened immune systems, and those with chronic lung, heart, kidney, or metabolic conditions may have a greater risk of severe respiratory illness.¹˒³˒⁵
Smoking and chronic respiratory conditions such as asthma or COPD can also make respiratory symptoms more significant.

We review when symptoms began, cough, congestion, sore throat, fever, breathing, chest symptoms, energy, sleep, recent exposures, and whether your illness is improving or worsening. Symptoms suggesting pneumonia, significant breathing difficulty, severe influenza or COVID-19, or another complication are referred for appropriate medical evaluation.
Your Recovery & Overall Health
For mild or recovering respiratory infections, we look at hydration, food intake, sleep, medications and supplements, activity, underlying health conditions, and factors that may be slowing recovery. Frequent respiratory illnesses can also be reviewed in context. Rather than assuming someone simply has a “weak immune system,” we consider factors such as repeated exposure, sleep, nutrition, smoking, chronic respiratory conditions, medications, and whether the pattern warrants further investigation.
Your Goals
Your priorities may include managing cough or congestion, maintaining hydration and nutrition, improving sleep while sick, returning gradually to normal activity, or understanding when lingering symptoms need further assessment. Your care plan is developed around the infection, symptom severity, health history, and goals.

Rest, adequate fluids, and symptom-directed care are sufficient for many uncomplicated viral respiratory infections.³ Antibiotics should be reserved for infections where a bacterial cause warrants them.⁶˒⁷
What to Expect
Initial Assessment
We begin with a detailed review of your symptoms, when they started, cough and breathing patterns, fever, recent respiratory exposures, medical history, medications and supplements, vaccination history when relevant, sleep, nutrition, and factors that may increase your risk of complications.
Respiratory Assessment & Medical Review
Many mild respiratory infections can be assessed from symptoms and clinical history without extensive testing. Depending on your presentation and risk factors, testing for influenza or COVID-19, a throat swab, chest examination, oxygen measurement, or chest imaging may be appropriate. Pneumonia and other lower respiratory infections may require further medical investigation.⁵˒¹⁰
Personalized Plan
Your care plan is developed around the likely infection, symptom severity, nutritional and hydration needs, medications, medical history, and goals. The focus is on appropriate symptom support and recovery rather than trying to “boost” the immune system.
Ongoing Support
Follow-up can help determine whether symptoms are resolving normally. A cough can linger after some respiratory infections, but persistent or worsening symptoms may require reassessment for pneumonia, asthma, sinusitis, post-viral airway irritation, or another cause.⁴˒⁶
When to Seek Support for Respiratory Tract Infections
Consider speaking with a healthcare provider if:
Symptoms are worsening rather than gradually improving
Fever is persistent or returns after initially improving
A cough continues for several weeks or is becoming more severe
You experience wheezing or recurrent chest symptoms
You develop significant sinus or throat symptoms that are not improving
Respiratory infections occur unusually often or recovery consistently takes much longer than expected
You have a chronic lung, heart, kidney, metabolic, or immune condition that increases your risk of complications
You may be eligible for early antiviral treatment for influenza or COVID-19¹¹˒¹²
Antiviral treatment for influenza works best when started early, ideally within about 48 hours of symptom onset, although treatment may still be used later in some higher-risk or severely ill patients.¹¹
Seek urgent or emergency medical care for:
Difficulty breathing or severe shortness of breath
Blue, grey, or unusually pale lips or skin
Significant or persistent chest pain
New confusion or difficulty staying awake
Coughing up significant amounts of blood
Severe weakness, fainting, or dehydration
Rapidly worsening illness
Severe symptoms in someone at high risk for respiratory complications³˒⁵
Frequently Asked Questions

Yes. Colds, influenza, COVID-19, RSV, and most cases of acute bronchitis are viral. Some respiratory infections, including certain cases of pneumonia, strep throat, and sinusitis, can be bacterial.¹˒⁶˒⁷

Not usually. Antibiotics do not work against viruses and therefore do not treat colds, influenza, COVID-19, or most uncomplicated cases of acute bronchitis. They are important when an appropriate bacterial infection is diagnosed or strongly suspected.⁶˒⁷

It depends on the infection. Many uncomplicated viral illnesses improve within one to two weeks, but symptoms such as cough or fatigue may last longer. Acute bronchitis commonly improves over two to three weeks, and its cough can sometimes persist beyond four weeks.⁴˒⁶

Cold symptoms often develop more gradually and commonly include congestion, runny nose, and sore throat. Influenza tends to begin more suddenly and is more likely to cause fever, significant fatigue, headache, cough, chills, and body aches.³

Pneumonia may cause cough, fever or chills, shortness of breath, rapid breathing, significant weakness, and chest pain that worsens when coughing or taking a deep breath. These symptoms should be medically assessed, especially when they are severe or worsening.⁵

No supplement has been established as a reliable substitute for measures such as vaccination, reducing exposure, staying home when sick, and appropriate medical care. Zinc may slightly shorten an existing common cold in some studies, but evidence is uncertain and it does not appear to reliably prevent colds.⁹

Not every infection can be prevented, but vaccination, staying home when sick, hand hygiene, improving indoor air quality and ventilation, and taking additional precautions around people at higher risk can reduce transmission and serious illness.²˒⁸ Annual influenza vaccination is recommended for eligible people, and COVID-19 and RSV immunization recommendations depend on age and individual risk.¹³˒¹⁴

Airways can remain irritated after a respiratory infection, so a cough may persist after other symptoms have resolved. A cough that continues to worsen, lasts unusually long, or occurs with shortness of breath, fever, blood, or chest pain should be reassessed.⁴˒⁶
