Overview
Joint and muscle pain, often described more broadly as musculoskeletal pain, can affect the muscles, joints, tendons, ligaments, bones, and surrounding tissues. Pain may develop suddenly after an injury or unfamiliar activity, gradually with repeated strain, or persist as part of conditions such as osteoarthritis, inflammatory arthritis, fibromyalgia, or chronic pain.¹˒²

What Is Joint & Muscle Pain?
The musculoskeletal system includes your muscles, bones, joints, tendons, ligaments, and other connective tissues that work together to support movement and physical function. Pain can arise from one or several of these structures and may be localized to a particular area or occur throughout several parts of the body.¹
Pain may be acute, such as after a muscle strain, sprain, or other injury, or it may become chronic. Pain that lasts or repeatedly returns for more than three months is generally considered chronic pain.² Chronic pain does not always mean that tissue is continuing to be damaged. Changes in pain processing, sleep, stress, activity, mood, and an underlying health condition can all influence how persistent pain is experienced.²˒³ Joint and muscle pain is therefore best understood by looking at the pattern, duration, location, associated symptoms, and effect on function, rather than assuming that all musculoskeletal pain has the same cause.

Musculoskeletal symptoms vary depending on the tissue involved and the underlying cause.
Joint symptoms may include:
Aching or pain
Stiffness
Swelling
Warmth or redness
Reduced range of motion
Pain during or after activity
Difficulty using or putting weight through the joint⁴˒⁵
Muscle and soft-tissue symptoms may include:
Aching or soreness
Tenderness
Tightness or cramping
Muscle spasms
Pain with particular movements
Weakness or reduced function

Widespread pain or tenderness in chronic pain conditions²
Persistent joint swelling, prolonged morning stiffness, or pain affecting several joints can suggest an inflammatory condition and deserves appropriate assessment.
Causes & Contributing Factors
Joint and muscle pain can develop for many reasons, and more than one factor may contribute at the same time.
Overuse, Strain & Injury
Muscle strains, ligament sprains, repetitive movements, sudden increases in physical activity, sports injuries, and other mechanical stresses are common causes of short-term musculoskeletal pain. How much activity a tissue is accustomed to can matter as much as the activity itself. A gradual return to movement is often more useful than prolonged inactivity once a serious injury has been ruled out.
Osteoarthritis & Joint Changes
Osteoarthritis is the most common type of arthritis in Canada and can cause joint pain, stiffness, and reduced mobility. Although structural changes occur within the joint, pain and physical function do not always correspond perfectly with what appears on an X-ray.⁵
Inflammatory & Autoimmune Conditions
Rheumatoid arthritis and other inflammatory arthritides can cause persistent joint pain, swelling, warmth, and prolonged morning stiffness. Early identification matters because medical treatment can help control inflammation and reduce the risk of progressive joint damage.⁴
Chronic & Widespread Pain
Conditions such as fibromyalgia can cause widespread pain alongside fatigue, sleep disturbances, stiffness, and changes in pain sensitivity. Chronic pain may also continue after an injury has healed or occur without one clearly identifiable tissue source.²˒⁶
Medications, Illness & Other Medical Conditions
Some medications and medical conditions can contribute to muscle aches, cramping, weakness, or joint symptoms. Viral illnesses may temporarily cause generalized muscle and joint aches, while certain medications can contribute to muscle symptoms in susceptible people. Medication-related pain should be reviewed rather than stopping a necessary medication without guidance.
Sleep, Stress & Pain Sensitivity
Poor sleep and prolonged stress can increase pain sensitivity and make recovery more difficult, particularly in people living with persistent pain. These factors can influence pain without necessarily being its original cause.²

Rather than assuming pain means you should avoid activity, the goal is often to find the right type and amount of movement for your current condition. Movement and exercise are important components of care for many musculoskeletal conditions, but recommendations should be adapted to the diagnosis and stage of recovery. For example, regular activity is strongly encouraged for osteoarthritis, while a hot, significantly swollen inflammatory joint may temporarily require modification of activity.⁵˒⁷

Initial Assessment
We begin with a detailed review of where your pain occurs, how it began, symptom duration, previous injuries, activity and exercise, joint stiffness or swelling, movement limitations, sleep, stress, medications and supplements, medical history, and how pain affects your daily life.
Pain & Musculoskeletal Assessment
From there, we look at your symptom pattern and relevant movement, strength, range of motion, tenderness, or joint findings when appropriate. The goal is to determine whether symptoms appear consistent with a mechanical or overuse problem, an established musculoskeletal condition, persistent pain, or something that requires further medical investigation.
Personalized Plan
Your care plan is developed around the likely cause or pattern of your pain, physical function, previous treatment, medical history, current activity level, and goals. Recommendations may include movement and rehabilitation strategies, acupuncture or physical medicine when appropriate, and support for sleep, stress, nutrition, or other factors relevant to recovery.
Ongoing Support
Follow-up focuses on changes in pain, mobility, strength, physical function, sleep, and your ability to return to meaningful activities. Persistent or worsening symptoms can prompt further assessment or referral rather than simply adding more treatments.
When to Seek Support for Joint & Muscle Pain
Consider speaking with a healthcare provider if:
Pain persists for several weeks or repeatedly returns
Pain significantly limits movement, exercise, work, or daily activities
A joint remains swollen or stiff
Morning stiffness lasts for a prolonged period
Pain affects several joints
Muscle weakness is persistent or worsening
Pain regularly disrupts sleep
Symptoms have not improved as expected after an injury
You develop widespread pain alongside ongoing fatigue or sleep difficulties⁴˒⁶
Seek prompt or urgent medical care for:
Severe pain following a significant injury
An obvious deformity or inability to use or bear weight through an injured limb
A suddenly hot, red, very swollen joint, particularly with fever
Severe or rapidly worsening muscle weakness
New numbness, significant weakness, loss of coordination, or difficulty walking
Loss of bowel or bladder control with new back pain or leg weakness
Severe muscle pain or weakness accompanied by dark or cola-coloured urine
Pain accompanied by fever, unexplained weight loss, or feeling seriously unwell
Shoulder, arm, back, neck, or jaw discomfort occurring with chest pressure, sweating, nausea, or shortness of breath⁹˒¹⁰

Joint pain is felt around the structures where bones meet and may be associated with stiffness, swelling, or reduced motion. Muscle pain is usually felt in the soft tissues and may involve soreness, tenderness, tightness, cramping, or pain with movement. The two can also occur together.

Persistent joint pain, swelling, reduced movement, or recurring stiffness may warrant an arthritis assessment. Prolonged morning stiffness, warmth, swelling, and symptoms affecting multiple joints can be particularly important clues for inflammatory arthritis.⁴

It depends on the cause and severity of the pain. A significant acute injury may initially require protection and activity modification, while prolonged complete rest can contribute to stiffness and weakness. For many chronic musculoskeletal conditions, appropriately progressed movement and exercise are important parts of treatment.⁵˒⁷

Stress can contribute to muscle tension and can influence how strongly pain is experienced, particularly when pain has become persistent. However, recurring or significant pain should not automatically be attributed to stress without considering injury, arthritis, medications, medical conditions, and other causes.²

Yes. Sleep and pain can influence one another, particularly with chronic pain conditions. Pain may disrupt sleep, while inadequate sleep can make pain feel more intense and reduce recovery and daily function.²

Not always. Many common musculoskeletal problems can initially be assessed through the history and physical examination. Imaging is most useful when the result is likely to change treatment or when symptoms suggest fracture, significant structural injury, infection, neurological involvement, cancer, or another serious condition.¹⁰

No. Inflammation plays an important role in conditions such as rheumatoid arthritis and some injuries, but pain can also arise from mechanical strain, osteoarthritis, nerve-related problems, chronic pain processing, medications, and many other causes.

Acupuncture may provide pain relief for some musculoskeletal conditions, with evidence varying according to the condition. It is generally used as one part of a broader treatment plan rather than as a replacement for appropriate medical or rehabilitative care.⁸
