High Cholesterol (Hypercholesterolemia)
Overview
High cholesterol, or hypercholesterolemia, usually refers to having elevated levels of cholesterol carried by certain lipoproteins in the blood, particularly low-density lipoprotein (LDL). Over time, higher exposure to LDL cholesterol contributes to cholesterol accumulation within artery walls and the development of atherosclerosis, increasing the risk of heart attack, stroke, and other cardiovascular disease.¹˒²

What Is High Cholesterol?
Cholesterol is a waxy, fat-like substance that your body needs to build cell membranes, produce hormones, and carry out other normal functions. Because cholesterol cannot travel freely through the bloodstream, it is transported in particles called lipoproteins.²˒⁴
A lipid panel commonly includes:
LDL cholesterol (LDL-C): Higher levels contribute to the development of atherosclerosis and cardiovascular disease.
HDL cholesterol (HDL-C): Higher levels are generally associated with lower cardiovascular risk, but HDL is not usually a treatment target on its own.
Non-HDL cholesterol: Represents the cholesterol carried by LDL and other atherogenic particles.
Triglycerides: Another type of fat in the blood that can provide additional information about metabolic and cardiovascular health.¹˒²
The broader term dyslipidemia refers to an abnormal lipid pattern, which may include high LDL cholesterol, elevated triglycerides, low HDL cholesterol, or a combination. There is not one cholesterol number that determines treatment for everyone. Lipid levels are interpreted together with a person's overall risk of developing cardiovascular disease.¹
Signs & Detection of High Cholesterol
High cholesterol usually causes no noticeable symptoms. You cannot feel LDL cholesterol building up in your arteries, which is why blood testing is important for identifying elevated levels before cardiovascular complications occur.²˒⁴ A standard lipid panel measures:
Total cholesterol
LDL cholesterol
HDL cholesterol
Non-HDL cholesterol
Triglycerides¹˒⁴
Additional tests can sometimes provide useful information. Apolipoprotein B (apoB)reflects the number of atherogenic lipoprotein particles and may be particularly helpful in certain metabolic patterns.Lipoprotein(a), or Lp(a), is largely genetically determined and is an independent cardiovascular risk factor. Measuring Lp(a) at least once in adulthood can help identify an inherited cardiovascular risk factor that may not otherwise appear on a standard lipid panel.¹˒⁵ Chest pain, stroke symptoms, or circulation problems are not symptoms of cholesterol itself. They may indicate cardiovascular disease that has developed over time and require appropriate medical assessment.
Causes & Contributing Factors
High cholesterol can reflect a combination of genetics, nutrition, metabolic health, medications, and other medical conditions.
Genetics & Family History
Your genes have a significant influence on how your body produces, processes, and clears cholesterol. Some people inherit familial hypercholesterolemia (FH), which can cause very high LDL cholesterol from an early age and substantially increase lifetime cardiovascular risk.¹˒² A personal or family history of very high cholesterol or premature heart disease can be an important reason to assess for inherited lipid disorders.
Nutrition
Dietary patterns can influence LDL cholesterol. In particular, replacing foods high in saturated fat with foods containing unsaturated fats and increasing soluble fibre can help lower LDL. Dietary patterns rich in vegetables, fruit, whole grains, legumes, nuts, seeds, and other minimally processed foods can also support overall cardiovascular health.⁶
Metabolic Health
Diabetes, insulin resistance, excess body weight when present, and other metabolic conditions can contribute to abnormal lipid patterns, particularly elevated triglycerides and atherogenic cholesterol.¹˒³
Physical Activity & Smoking
Physical inactivity and smoking contribute to overall cardiovascular risk. Regular physical activity and avoiding tobacco are important parts of cardiovascular prevention, whether or not medication is also required.³˒⁶
Other Medical Conditions & Medications
Thyroid disease, kidney disease, liver conditions, and some medications can alter cholesterol or triglyceride levels. Identifying these contributors may be part of the assessment when lipid levels change unexpectedly.

Soluble fibre, including foods such as oats, barley, legumes, and psyllium, can help reduce LDL cholesterol. Plant sterols can also lower LDL when consumed in sufficient amounts.⁶ These strategies can complement medication but should not be used to replace cholesterol-lowering therapy when medication is indicated.

Lipid & Cardiovascular Risk Assessment
We review your LDL, HDL, non-HDL cholesterol, and triglycerides alongside your broader cardiovascular risk. ApoB may provide additional information in selected situations, and Lp(a) can be measured to identify an inherited cardiovascular risk factor that may not otherwise appear on a standard lipid panel.¹˒⁵ Very high LDL cholesterol, particularly LDL-C of 5.0 mmol/L or higher, can indicate a high-risk lipid pattern and should raise consideration of familial hypercholesterolemia.¹˒⁸

When to Seek Support for High Cholesterol

Have experienced a heart attack, stroke, or other atherosclerotic cardiovascular condition¹˒²
Very high LDL cholesterol or a strong family history of early cardiovascular disease deserves medical assessment for a possible inherited lipid condition.¹˒⁸
Seek urgent medical attention for possible signs of a heart attack or stroke, such as new chest pressure or pain, sudden weakness or numbness, difficulty speaking, sudden severe shortness of breath, or other acute cardiovascular symptoms.

Usually not. High cholesterol is generally detected through blood testing because elevated LDL itself does not cause noticeable symptoms. Cardiovascular symptoms may appear only after atherosclerosis has already caused complications.²˒⁴

LDL carries cholesterol in particles that can contribute to plaque formation in artery walls when present at elevated levels. HDL is associated with transporting cholesterol through the body and is generally associated with lower cardiovascular risk, but treatment focuses primarily on reducing atherogenic cholesterol rather than simply trying to raise HDL.¹˒²

Very high LDL, such as 5.0 mmol/L or above, is an important high-risk finding that may warrant cholesterol-lowering treatment and assessment for familial hypercholesterolemia.¹˒⁸

Lp(a) is a genetically determined lipoprotein that can independently increase cardiovascular risk. Because levels are largely inherited and remain relatively stable, measuring Lp(a) once in adulthood can help identify risk that may not be apparent from a standard cholesterol panel.⁵

Yes. Replacing saturated fats with unsaturated fats, increasing soluble fibre, and following an overall heart-healthy eating pattern can reduce LDL cholesterol. The amount of improvement varies between individuals, particularly because genetics can have a strong influence on cholesterol levels.⁶

Usually not. Overall eating patterns and the types of dietary fat consumed are generally more important than eliminating every food containing cholesterol. An individualized approach may be appropriate when LDL levels are particularly high.⁶

Lifestyle changes can meaningfully improve cholesterol and cardiovascular health, but they are not always sufficient on their own.
